Requirements

• E28A: Passport/travel document valid at least 6 months; route-specific non-national document exceptions apply.
• E28A: Recent colour photo taken within previous year.
• E28A: Last 3 months bank statements showing at least USD2,000/equivalent; applicant/guarantor account where permitted.
• E28A: CV/personal history.

  The provider preparation checklist asks for passport validity of at least 18 months. Confirm the selected duration with Atelix; the official route minimum is listed separately.

  Scan the complete passport identity page, including the photo, passport number, issue/expiry dates and machine-readable lines at the bottom. Keep every edge visible and avoid glare.

  Your passport must have at least six months of validity. Use the same passport and spelling as your application.

• Recent colour photo

  Prepare a colour photograph on a red background for this provider checklist; retain an original without filters or edits.

  Use a recent colour portrait, facing the camera, with a neutral expression and no blur, filters or objects covering your face.

  Prepare JPG/JPEG or PNG, at least 400 × 600 pixels and no more than 2 MB for government filing. Keep the head and chin within the frame; the head should occupy about 50–60% of the image height.

  Use a photograph taken within the last year. Atelix accepts originals up to 10 MB, but the government photo limit is smaller.

• Personal bank statements

  The referenced bank evidence calls for at least USD 2,000 or equivalent; show the account holder and complete statement period.

  Upload bank-issued statements for the latest three months. Include all pages showing your account-holder name, statement period, currency, transactions and closing balance.

  Use complete monthly statements downloaded from your bank rather than a cropped balance screenshot. Keep the name consistent with your passport.

• Shareholding evidence

  Provide company records proving your ownership of at least IDR 10 billion in shares for E28A.

  Upload the deed, shareholder record or current company profile showing your name, share value and company identity. Include amendments if they establish your current holding.

• Company establishment approval

  Upload the company establishment deed and its Ministry approval. Include the current company profile or amendments needed to identify the legal entity.

  The company name must match your investment and sponsor records. Preserve official approval numbers and all pages.

• Company bank statements

  Upload the company’s bank-issued statements for the latest two months, including all pages.

  Keep the company account-holder name, currency, statement dates, transactions and balances visible. The company name must match its establishment records.

  Upload the visa application letter and signed guarantee issued by your responsible sponsor. Include the applicant’s passport name and the sponsor’s identity or company name.
